Connect 2 Care (C2C) is a program that links people living with HIV into medical care.  C2C works with HIV-positive individuals who are newly diagnosed or who have fallen out of medical care.  C2C is a mobile program so we can meet individuals wherever is easiest.  C2C is a short-term, time-limited program striving to provide support to clients in accessing medical care and provides referrals to other important services like housing, case management etc.  Incentives are available if you are eligible to participate in the evaluation of C2C.
