 One of the most important components of Chicago House is our Community Case Management Program, which helps people with HIV and AIDS live independently in their own homes and remain members of the Chicago community for as long as possible.
Case managers help the clients establish short- and long-term goals and link them with the appropriate community-based support services that will turn these goals into reality. Specifically, the case managers help clients locate and make use of a full range of services, including social service programs and support services; psychological and substance abuse support groups; food and nutrition programs; and legal services that assure that people with AIDS will be treated fairly in matters of insurance coverage, employee rights and permanent housing.
Most important of all, case managers provide counsel and encouragement and help our clients deal successfully with day-to-day life in the larger Chicago community.
