Scattered-Site Housing

Scattered-site housing finds homes for HIV-positive individuals through rent subsidy agreements with landlords. This enables Chicago House to expand our housing capacity through existing apartments.

Hospital to Housing Program (HHP)

The Hospital to Housing Program serves homeless individuals with chronic illness by moving them into permanent supportive housing from the hospital and providing intensive case management services. The program works with individuals in increasing life and tenancy skills in order to ensure housing stability.

The Samaritan Housing Program

The Samaritan Program serves individuals who are disabled and homeless. Referrals are received from local hospitals, housing programs, and/or street outreach. Upon accepting a referral, Chicago House quickly provides intensive case management to meet the specialized needs of the client(s) and places them in an apartment they can call their own. A harm reduction model is utilzied to help clients explore alternatives to drug and alcohol use while educating them on the risks of transmitting HIV to their partners.
