Housing Opportunities for Youth Program Manager

Chicago House is seeking a Program Manager to work with a new housing program, HOY (Housing Opportunities for Youth). The HOY Program Manager is part of the Housing & Health Navigation Department. The HOY program comprises a mix of Rapid Rehousing and Transitional Housing units that operate within the spirit of the Housing First model and are designed to reduce housing instability amongst youth ages 18-24. HOY participants engage with a multidisciplinary care team on housing, health, and income goals, to facilitate their ability to financially sustain an apartment prior to program discharge. The HOY Program Manager supervisees the HOY Housing Case Manager and will work closely with other social service providers in the department and agency to coordinate wrap-around services. Supportive Housing Case Manager


The Retention in Care Case Manager (RCCM) resides within the Housing & Health Navigation department and is a part of the Housing & Health team. They will employ a health-system based and integrated health and social care service delivery model to improve overall medical, behavioral health, and social care outcomes for unstably housed people living with HIV and those vulnerable to HIV-acquisition. The RCCM is part of a team that consists of medical and non-medical care staff. They may be required to work non-traditional business hours to meet the demands of the program. The RCCM’s primary role is to provide case management services focused on social care to support the rapid linkage and retention in integrated health and social care for unstably housed people who are newly diagnosed, out of care, at-risk of HIV, and those who are chronically disengaged from medical care and housing supports. Comprehensively, they will be addressing the multiple barriers to accessing and staying retained in integrated health and social care. To apply, email a resume and cover letter to Mikey Brown.
