Housing


Chicago House provides housing and resources for low-income individuals and families who have experienced chronic homelessness or unstable housing and are living with or vulnerable to HIV.


95% of individuals in our Residential Housing programs do not return to homelessness.

87% of people living with HIV in Chicago House residences show an undetectable HIV viral load after a minimum of 6 months in stable housing.

81% reported improved mental health in the last 6 months.



 
 

We believe a home is a key determinant in reaching and maintaining positive health outcomes and greater well-being.


Chicago House programs strive to improve the health and well-being of those we serve, improve self-sufficiency, and provide access to essential services. Through both internal and external referrals, Chicago House provides the following wrap-around services to individuals in all of our housing programs: linkage to HIV care, linkage to PrEP, employment services, case management, after-school programming, mental health services, and legal services for transgender clients.

We focus on two types of housing:
Community-Based Housing
Residential Housing

 

Community-Based Housing

Our Community-Based Housing program provides autonomous, independent living solutions in subsidized rental units across the Chicagoland area for individuals and families who are referred to us via the Coordinated Entry System or via internal waitlists. We operate under the Housing First approach and believe a stable home is an essential foundation in reaching and maintaining positive health outcomes and greater well-being.

Individuals and families impacted by HIV/AIDS and other chronic illnesses who are experiencing homelessness or housing instability receive housing information services, which includes navigation of housing-related goals and resources in Chicago, and are connected to additional Chicago House services, including: case management, pharmacy, employment and legal services.

Our goal is to create and maintain permanent housing solutions while improving the health and well-being of the individuals we serve.

Residential Housing

Our Residential Housing program serves single adults living with HIV in efficiency or studio apartments, as well as 1-3 bedrooms for families impacted by HIV. Residents have on-site access to Chicago House services and support staff, including: case management, medical care coordination and regular programming and activities.

The Residential Housing program is primarily dedicated to serving people living with or most vulnerable to HIV.
