Moment #10: Summer 1994 - Chicago House pilots Community Case Management Program, expanding support services to non-residents

With generous funding from the AIDS Foundation Chicago (AFC), Chicago House was able to offer its support services to non-resident community members. This expansion laid the foundation for the way Chicago House operates today, delivering wraparound services to our residents while providing case management, drop-in services, and linkage to care for thousands of individuals who do not currently live in our three residential facilities.
